Terms and Conditions for Manela Health Concierge Membership

1. Acceptance of Terms

These Terms and Conditions (“Terms”) govern your use of the Manela Health website, landing pages, forms, and related services operated by Manela Cardiology PLLC d/b/a Manela Health (“Manela Health,” “we,” “us,” or “our”). By accessing this website, submitting an inquiry, or otherwise using this site or related services, you agree to these Terms.

2. Purpose of This Website and Landing Page

This website and related landing pages are intended to provide general information about Manela Health and its concierge membership program and to allow prospective patients to inquire about membership. Submitting an inquiry, form, or request for information through this website does not create a physician-patient relationship, does not guarantee acceptance into the membership program, and does not constitute the purchase of membership or medical services.

4. Concierge Membership Overview

Manela Health offers a concierge medical membership program designed to provide enhanced physician access, personalized care, and a more proactive approach to health and wellness. The concierge membership term is 12 months from the date of enrollment. The current annual membership fee is $4,800 unless otherwise stated in writing by Manela Health. The annual membership includes high-quality primary care services, enhanced physician access and communication, longer and more personalized visits, advanced cardiovascular evaluation and diagnostics as outlined by the practice, body composition analysis, and preventive and functional medicine consultation options where applicable. Descriptions of membership features on the website or in marketing materials are for general informational purposes only and may be updated from time to time.

5. Inquiry Only — No Online Purchase Through This Page

This landing page is for informational and inquiry purposes only. You are not purchasing a membership, medical service, or treatment by submitting your information through this website. Any membership enrollment, financial obligations, treatment consent, arbitration agreement, and patient documentation will be addressed separately if you are accepted into the program.

6. Physician Approval and Limited Availability

All memberships are subject to physician approval, clinical appropriateness, and practice availability. Submission of an inquiry does not guarantee acceptance. Manela Health reserves the right, in its discretion and as permitted by law, to accept, decline, postpone, or discontinue consideration of any membership inquiry based on clinical fit, panel capacity, operational limitations, or other lawful reasons.

7. Renewal Policy

Memberships do not renew automatically. Approximately one to two months before the end of the membership term, Manela Health may meet with the member to review the prior year, discuss satisfaction with care and outcomes, and determine whether renewal is appropriate. Renewal requires the patient’s active consent, completion of any renewal documentation, and payment of the applicable renewal fee. There will be no automatic charge, rollover, or renewal without the patient’s express agreement.

8. Cancellation and Refund Policy

Upon initial enrollment, there is a limited cooling-off period of 14 days during which a member may request cancellation, provided that no substantive clinical services have begun. Substantive clinical services include a comprehensive intake visit, development of a care or management plan, ordering of laboratory testing or imaging, initiation of testing, or initiation of treatment programs. Once any substantive services have begun, the membership is committed for the full 12-month term and is non-refundable for that membership period. Exceptions, such as relocation out of state or severe illness preventing participation, may be considered at the sole discretion of Manela Health, but are not guaranteed. Because this website is inquiry-only and does not process membership purchases, no refund is created or owed solely as a result of submitting information through this page.

9. Services Included and Services with Additional Charges

The annual membership fee includes the professional services specifically described by Manela Health as part of the concierge program, including primary care and ongoing management, enhanced access including longer visits and direct communication as defined by the practice, cardiovascular risk assessment and diagnostics as outlined by the practice, body composition analysis, and professional consultation relating to preventive and functional medicine options. Certain tests, treatments, and services may involve separate charges in addition to the annual membership fee. Additional services billed separately may include transcranial magnetic pulse therapy for mental and neurologic health, body sculpting or aesthetic treatments, pelvic floor muscle-building sessions, shockwave therapy for pain, platelet-rich plasma injections, facial aesthetics and regenerative procedures including RF microneedling and targeted ultrasound, outside laboratory services, outside imaging services, supplements, medications, specialist consultations, and hospital-based services. Fees for separately billed services will be disclosed before treatment when applicable and may require separate consent.

10. Payment Terms

Membership fees may be paid in full upfront or financed through Cherry Financial, if approved and offered by the practice. If financing is used, monthly payments may be made to Cherry Financial, and finance charges or fees may apply according to Cherry Financial’s terms. Any financing arrangement is between the patient and the financing provider and is subject to that provider’s separate agreement. Manela Health is not responsible for the terms, approval decisions, finance fees, or repayment obligations imposed by third-party financing providers.

11. Insurance and Out-of-Network Reimbursement

The concierge membership is not health insurance and does not replace health insurance. Patients remain responsible for maintaining any health insurance coverage they consider appropriate. Membership fees generally cover enhanced access and specified services offered by Manela Health, but do not include hospitalizations, emergency services, specialist care, or outside laboratory or imaging services, which are billed separately by those providers. In some circumstances, a patient may be able to seek reimbursement from a PPO plan for certain covered out-of-network services. Any such reimbursement is not guaranteed and depends entirely on the patient’s insurance plan and the insurer’s policies. Patients are solely responsible for verifying their own insurance benefits and reimbursement eligibility.

12. Communication and Access

Patients may communicate with the practice by phone, text, or email using practice-approved contact channels. After-hours communications may be directed through the office number and may be handled by the physician, depending on the nature of the issue, physician availability, and practice procedures. While Manela Health seeks to provide responsive access for urgent matters, communication channels are not intended for life-threatening or emergency situations.

13. Emergency and Urgent Care Disclaimer

Manela Health may be available to assist with urgent medical matters as part of the concierge care model. However, the membership and its communication channels are not a substitute for emergency medical care. If you are experiencing a true emergency, call 911 immediately or go to the nearest emergency room. Do not rely on website forms, email, text messages, or non-emergency communication channels for urgent or life-threatening medical situations.

14. No Medical Advice and No Physician-Patient Relationship from Website Use
